2. Employer will file and report the injury to the company’s workers’ comp carrier.
3. A claim for your injury will be created. Your employer will provide you with the details of your claim.
4. Your employer or claims manager will direct you to see a treating doctor for your injury.
5. If you are requiring a specialist’s evaluation for your injury, your treating doctor will refer you to one.
*Please note that our doctors at Dallas Orthopedic and Shoulder Institute are considered specialists, not treating doctors.

1. Referral from your treating doctor. The referral can be faxed to 214-256-3770 or “Request an Appointment” by clicking on the orange button above.
2. Demographics and claim Information such as:
Basic Demographics: Name, DOB, Address, Phone Number, SSN
Employer/Company Info
Name of workers’ comp insurance carrier
Claim number Claim manager’s name and phone number
Injured body part and date of injury
3. Immediately after we receive the information that we need, we will contact your claim manager to get approval for evaluation or treatment.
4. Once approval is obtained, we will reach out to you for scheduling.
